We learned with sadness that Caroline Kawa (aka Care Failure), the lead singer of the band Die Mannequin, had passed away less than one month before her 37th birthday today, Thursday, March 30, 2023. Unfortunately, destiny had planned something else for the talented vocalist.
Nathan Clearihue posted the sad news of Die Mannequin’s Care Failure’s passing on Facebook.
“R.I.P Caroline Kawa otherwise known as Care Failure from Die Mannequin. Saw them many times and of course, they filmed Hard Core logo 2 here in Regina. She was a wonderful human being. Gone way too soon 🤘🤘,” he wrote.
Cause of Death- Caroline Kawa
Caroline Slezak Kawa, widely known as Care Failure of Die Mannequin, died suddenly this past Thursday. Her death was highly unexpected and no cause of death was released at the time of this writing. She was only 36 years old.

Her friend Kerry Madan Posted a deepest condolence message on Facebook. It says,
Please keep Caroline Slezak Kawa (Care) the beloved niece of my dear friend, Ellen Slezak, in your hearts. I saw Care play at the Viper Room years ago with her band, Die Mannequin, and her wildly radiant energy was electrifying along with her tremendous kindness, generosity, and lovely spirit as she greeted us all after the show. This is an exquisitely hard time for this beautiful family, and Care’s father says it best here. “Please be kind and respect all the very different people you may bounce into each day. That was Care’s way.”

Who was Caroline Kawa a.k.a Care Failure?
Caroline Slezak Kawa was a Canadian woman born on April 30, 1986. She went on to become the lead vocalist of the Canadian alternative rock band Die Mannequin as “Care Failure.” She was based out of Toronto, Canada.
Die Mannequin toured across Canada several times, opening for Buckcherry, Guns N’ Roses, Marilyn Manson, and Sum 41. They have also toured Europe on several occasions, alone and as a successful opening act for Danko Jones in 2008.
Apart from being a founding member of Die Mannequin “Care Failure” also worked with Big Dirty Band. She also appeared in the movie “Hard Core Logo 2.” Caroline launched her album “Unicorn Streak” in 2007 which is a collection of ten songs by her band.
Die Mannequin Band
Die Mannequin is an alternative rock band from Toronto, Ontario, Canada fronted by guitar player and singer Care Failure (born Caroline Kawa). They played the “Next Big Thing” Stage at Edgefest 1 in 2006, have toured across Eastern Canada several times on their own as well as opening for Buckcherry on their Canadian tour.
Die Mannequin also opened for Guns N’ Roses on the Canadian dates of their 2006 North American Tour and supported Sum 41 on their Strength in Numbers tour during the fall of 2007. They have toured Europe on several occasions, alone and as an opening act for the European Danko Jones tour in 2008. They are also playing one of the legs of the Jägermeister tour with Bloodlights and Oceansize. In the fall of 2009, Die Mannequin opened for Marilyn Manson on the full Canadian leg of his “The High End of Low” tour in addition to a cross-Canada tour of their own featuring the opening act Raggedy Angry.

So sad to see this today. Not much in the mainstream media regarding her death but Caroline Kawa (aka Care Failure) was an amazing musical artist. Her band Die Mannequin was amazing and I was a big fan. She didn’t hide her past drug and alcohol addictions and was amazing at sharing her struggles.
Her cause of death hasn’t been made public but her struggles with addiction and mental issues were well known.
